Memberships live in the creator studio. Open your profile menu and choose Memberships — that page is where you apply, and where you manage everything afterwards.
The requirements are re-checked when you submit, not only when the page loads. A visible button is not the same as being eligible.
The same page gains a tier editor. Describe each tier, choose its benefits, and send it for review. A tier goes live only after that second review — approval of your application is not approval of your tiers.
You set neither the names nor the prices. Both tiers are called the same thing and cost the same on every creator, so a member reading a badge always knows what it means.
